Wasted $2,800 on a Facebook ads agency that just ran the same 3 ads for 4 months

I hired a specialized agency back in May to handle our lead generation. They sent me pretty reports showing cost per lead dropping month over month. Then a client mentioned seeing the exact same ad creative in July that ran in May. I checked the backend and they had duplicated one winning ad 12 times with different headlines. No new testing, no audience changes for 16 weeks. Anyone had to untangle a mess like this with a subcontractor or white label partner?

Did they ever explain why they stopped testing new creative, or did they just say the cost per lead was the only metric that mattered?
