Had a weird injector cup failure on a 6.0 Powerstroke last Thursday

It was on a 2005 F-250 that came in for a coolant leak. Pressure test showed it was coming from the #3 cylinder, which is pretty unusual. Pulled the valve cover and sure enough, the cup was cracked right at the base. The truck only has 140k miles, and the owner said he'd been using the Ford coolant. Took me about 6 hours total to get the old one out and the new one in. Has anyone else seen these fail without the usual signs of cavitation or bad coolant?

Heard from a buddy who runs a diesel shop that this happens sometimes when people use the blue Ford coolant but don't flush it right. He said the additives drop out of suspension if you mix it with any leftover green stuff and it turns into this gritty sludge over time. That crap settles right at the injector cup base and starts eating into the metal, even with low miles. I'd check the coolant history closer maybe ask the owner if he ever topped it off with something from the auto parts store. Just a thought based on what I've seen in forums and from that guy's experience.

Honestly, my old truck gets the same way if I even look at another vehicle. I swear it hears me compliment a neighbor's new paint job and then decides to have a weird electrical issue the next morning. It's like having a jealous partner that guzzles diesel instead of wine.
