Token naming inconsistency ate 3 days of my life last month

Our team spent 3 full days in Chicago tracing a spacing bug that only showed up in production because someone named a token 'space-md' in Figma but the dev file had 'spacing-medium', and it took a designer from another project to spot the mismatch, so has anyone else built a naming convention that actually sticks across teams or do we all just accept this chaos?
