The moment our entire color system broke because one designer saved a hex code without the hash

I was sitting in a coffee shop on Broadway in Nashville when I got the alert that every button on our app went black after a Friday deploy, and the fix took 14 hours of manually checking 230 tokens because nobody had set up a hex validation rule in the pipeline, so do you put your trust in tooling or in people double-checking their work?

Lost a whole product launch the same way. Software needs checks now.

Did you not have any backup or rollback plan before you pushed it live? I find it hard to believe you just had zero safety nets for a whole product launch. There's usually at least some kind of manual checklist or a way to test things in a staging environment first. What exactly went wrong that a simple software check would have caught?
