Rented excavator bucket came off on a job in Tulsa yesterday

The pin had a hairline crack nobody saw during the pre-inspection, and it let go mid-scoop in front of the homeowner. I patched it with a grade 8 bolt and a spare washer, but now I keep wondering how often rental gear gets skipped on checks like that. Has anyone else had rental equipment fail in a way the rental yard refused to cover?
