Why does nobody talk about the 'active' state on form buttons being just a color change?

Honestly, I just watched a user with red-green color blindness at my office in Chicago try to submit a form three times because the only sign the button was working was it turned from green to a slightly different green.

That's a huge accessibility fail. Color alone should never be the only signal for a state change. Good design needs a clear shape shift or text update too. It's lazy and leaves so many users behind. Watching someone struggle like that makes the problem painfully real. Basic usability testing would catch this.
