Tool offset drift cost me 6 hours on a simple part

Ran a batch of 50 flanges in Toledo last Tuesday and the finish started wandering halfway through, turns out my tool presetter was off by 0.004" from a dirty seat. Took me 4 hours of re-cutting and 2 hours of cleaning to figure out it wasn't the holder or speeds, just a spec of swarf. Anyone else ever chase a phantom tolerance like that?
