Ran a giant banner with grayscale photos by accident, client spotted it at the venue

Last Friday I sent a 12 foot banner to a trade show in Denver and forgot my linked Photoshop file was still in grayscale. The printer output looked fine in the PDF proof because I had it set to grayscale print preview, so I approved it without checking the embedded color profile. Turned out every photo on the banner was black and white while the text was full color, looked like a memorial poster. The client called me from the expo floor and asked if their new product launch was a tribute to something. Has anyone else trusted the PDF proof and got burned by a hidden color mode issue?
