Miter saw blade walked off mid-cut, almost lost a finger in Austin

Last Thursday the blade nut on my old Dewalt came loose during a crown molding cut, the blade wobbled and chewed up a $40 piece of oak. I now check torque on that nut before every single use, anyone else had a tool suddenly throw a fit after years of fine service?
