The color profile mismatch that wrecked a full print run in Portland

I designed a menu for a small cafe and sent the file as RGB, thinking the printer would convert it. Every single color came out muddy and dull, and the owner was not happy with the 200 copies. After that I learned to always embed CMYK and check the soft proof twice, even if it adds 10 minutes. Has anyone else had a print job fail because of a sneaky color mode issue?
