Accidentally used wax thread for a Coptic stitch...
Grabbed the wrong spool from my drawer last week and didn't realize until I was three sections in. That stuff just does not hold a knot. The whole spine came loose after I cut the tails. Learned my lesson - gotta keep my waxed and unwaxed thread in separate boxes now. Anyone else mix up their thread and ruin a project?

Stop using waxed thread for Coptic then.

Hold on, is there really a right or wrong thread for Coptic stitch? I've been binding books for over a decade, and here's the thing - waxed thread has its place. The trick with waxed thread is you have to melt the knot slightly with your fingernail or a bone folder to get it to hold. A simple tug after the knot is tied won't do it. I've used waxed thread on purpose because it's much smoother going through the spine folds and doesn't fray or get fuzzy as you work. I'd say give your current book a second chance before you toss it. Tie off those tails with a bit more heat or pressure on the knot, and it might surprise you.
